High School Youth

We invite all high school teens to join the High School Youth group.

If you are interested in high school youth group, please fill out the high school youth group registration form, generic permission slip, annual medical release and the high school survey.

If you want to register for confirmation, you will need to fill out the confirmation registration, medical release form and generic permission form.

If you are registering for both high school youth group AND Confirmation, you only need one medical release and one generic permission form.
